Applications for Round 2 due

Application packets are due no later than 3 p.m. on August 25, 2023.
Please be sure to complete, sign and date all forms.

1. Packet checklist 
2. Packet application (please type or print neatly)
3. Statement of significance (not to exceed two full pages, follow format; include form on page 7 of application)
4. Letters of Intent* 
5. Photographs, mock-ups and/or other examples of sign to be installed (with verbiage)
6. Map and photograph to show placement of proposed marker

*If organization is applying on behalf of the municipality, a "Letter of Intent" is required from the town if sign(s) are intended to be installed on municipal property. If municipality is installing sign on private property, letter from property owners.

Please submit in one of three ways to Gina.
