Are you unable to prepare a meal for yourself?  Live alone?  

You can receive a hot meal delivered 5 days a week, if you are homebound or unable to leave your home, with the exception of going to medical or other essential appointments.

You must be without at-home support from family, friends, or neighbors to help you get a daily meal.

We will send your doctor an authorization from which must be signed by the doctor stating you need a home-delivered meal.
