Jersey Assistance for Community Caregivers (JACC)
Jersey Assistance for Community Caregivers (JACC) is a program that provides in-home services to seniors at risk of placement in a nursing facility. JACC includes an array service designed to supplement assistance given by the individual's caregiver network of family, friends, and neighbors.
By designing service plans uniquely tailored to the individual, JACC works to enhance the individual's community care options. JACC also strengthens the ability of caregivers to continue in their vital role as primary support providers.
Is JACC a Medicaid Program?
No, JACC is not a Medicaid Program. JACC serves individual who are not eligible for Medicaid or Medicaid waiver services. JACC is a program supported totally with state funds.
Services
Based on results of pertinent assessments, a Plan of Care (POC) is developed collaboratively by the participant and his/her care manager. This POC specifies the services to be delivered, which may include:
- Case Management
- Respite Care
- Homemaker Services
- Environmental Accessibility Modifications
- Personal Emergency Response Systems
- Home-Delivered Meal Service
- Caregiver/Recipient Training
- Special Medical Equipment and Supplies
- Transportation
- Chore Services
- Attendant Care

Applications for services under JACC are processed through the county NJ EASE site, which can be reached by dialing 1 (877) 222-3737.
Limits to the Services an Individual May Receive
JACC services are limited to $1,090 per month.
Eligibility to Receive These Services
A JACC eligible individual is a New Jersey resident who:
- Is 60 years of age or older
- Resides in a home which he/she owns, rents, or is the home of a relative, that is not licensed
- Is financially ineligible for Medicaid or Medicaid waiver services
- Has countable resources at or below $40,000 for an individual, or $60,000 for a couple
- Has no alternate means available to secure needed services and/or supports
- Has been determined to be clinically eligible for nursing facility level of care
- Is a United States citizen or a Qualified Alien
- Has a countable income that is less than 365% of the Federal Poverty Level
Co-Pay
JACC participants will contribute to the cost of their services. The co-pay obligation is based on countable income applied to a sliding scale. Co-pay is payable directly to the designated Billing Agent provider of services, who will bill the JACC participant.
JACC Providers
Prospective vendors can apply to provide services to participants of the JACC program. All service providers must apply and meet certain qualifications set by the Department of Human Services. Once enrolled, providers can be reimbursed for service delivery to program participants.
