Warren County's Community Health Improvement Plan
Warren County’s Community Health Needs Assessment 2019
The State of New Jersey requires all counties to conduct a Community Health Needs Assessment (CHNA) and develop a corresponding Community Health Improvement Plan (CHIP).
This edition of the Warren County CHIP was developed in partnership with the Warren County Community Health Improvement Coalition (WCCHIC), a group of government agencies, local non-profit partners, and additional community stakeholders.
The 2019 Warren County CHNA and CHIP highlight a data-driven approach to determining the health status, behaviors, and needs of our resident population. The process to complete this health assessment and plan has involved input from a variety of community constituents, both public and private stakeholders, and residents of all ages. This report includes primary data from the Community Voice Survey, secondary data analysis conducted by the North Jersey Health Collaborative (NJHC) Data Committee, and other data sources including the New Jersey Department of Health (NJDOH), the Centers for Disease Control and Prevention (CDC), and other research institutions.
The strategies identified in the plan will help WCHD and its WCCHIC partners address the major health challenges facing the residents of the county. WCHD looks forward to working on a variety of initiatives and supporting local partners to promote health and wellbeing for all Warren County residents.
